Tyler ContinueCARE Hospital has updated its Visitor Policy, based on CMS/CDC recommendations, local prevalence of the virus, and to provide a safe environment for everyone. This policy is subject to change based on incidence of virus. Effective March 24th, 2021
Non-Covid Patients: Visitation Hours 5:00am-7:00pm
- No more than 2 visitors on the unit at a time per patient; We strongly encourage you to limit the number of total visitors but will allow visitors to alternate
- Visitors may be further limited based on patients condition and risk status
- Visitors must wear facemask at all times, including while in the patient room
- All visitors must pass screening at the front door of the main hospital for s/s of illness.
Covid Patients: by appointment only from 11am-1pm daily
- 1 designated visitor per hospitalization
- Other considerations will be made for patients at end of life
- Must pass covid/illness s/s screening prior to arrival
- If intubated/non aerosolizing: 15 minute visit in room
- If not intubated/aerosolizing: 15 minute window visit

Tyler ContinueCARE Hospital is aware of the Executive order issued by Governor Abbott lifting mandatory masking effective March 10, 2021. While we respect the Governor’s order, we have an obligation to continue to provide a safe care environment for our patients, employees, and visitors and will continue to require masking while in our facility based on current CDC guidelines.
All employees and visitors will still be required to mask at all times while in our facility. We are continuously evaluating our policies based on CDC recommendations and will update as necessary in an effort to provide safe and effective care. As our community increases the number of vaccinated individuals and the prevalence of COVID-19 shifts locally we realize further decisions will need to be made and we will communicate any changes as they occur. Tyler ContinueCARE Hospital opened in May 2004 as a 51-bed Long Term Acute Care Hospital (LTACH). We are located on the fourth floor of our host hospital, CHRISTUS Mother Frances Hospital – Tyler. We enjoy the best of both worlds by having a relationship with a large 450-bed acute-care hospital yet maintaining our small hospital atmosphere. Our employees are a dedicated team who work together to deliver the best possible care for our patients. Since we have a longer length of stay, our staff has the opportunity to know patients and families. We believe spending quality time makes a difference. And we want to make a difference.
As a “hospital within a hospital,” we are a separate yet fully licensed acute-care hospital with the ability to provide intensive and complex medical treatment. Our specialty programs are specifically designed to meet the needs of long-term, acute-care patients.
CIHQ Accreditation and Center of Excellence
In 2016, Tyler ContinueCARE Hospital received a three-year accreditation by the Center for Improvement in Healthcare Quality, as well as designation as the first CIHQ “Center of Excellence” LTACH in Texas. “Not every community is fortunate to have a long term acute care hospital to care for patients with complex medical conditions, ” said Stephanie Hyde, RN, MSN, CEO of Tyler ContinueCARE Hospital. “We provide high quality care in a compassionate environment for very sick patients – every day. This designation reflects our commitment to providing the highest level of quality healthcare to our community.”
The hospital was recognized with an Excellence Through Insight award for “Overall Physician Satisfaction” by HealthStream, Inc. in 2014 and 2015, which exemplifies its patient-first philosophy and ‘Sacred Work’ culture to do the right thing.
Since opening, Tyler ContinueCARE Hospital has served more than 6,000 patients from a 100-mile radius.
