Community Health Needs Assessment

Community Health Needs Assessment

As part of the Affordable Care Act, tax-exempt hospitals are required to conduct a Community Health Needs Assessment (CHNA). In 2013, a CHNA was conducted for Carolinas ContinueCARE Hospital at Pineville, previously known as Carolinas Specialty Hospital, that identified the greatest health needs of a defined primary service area (Cabarrus, NC, Gaston, NC, Mecklenburg, NC, Union, NC and York, SC) and secondary service area (Chester, SC, Cleveland, NC, Iredell, NC, Lancaster, SC, Lincoln, NC, Rowan, NC and Stanly, NC). In 2017, Carolinas ContinueCARE Hospital at Pineville conducted its second CHNA report which includes a comprehensive assessment of Mecklenburg County, NC.

The analyses include a careful review of the most current health data available, demographics and input from numerous community representatives. The process culminates in the development of an Implementation Plan to address the significant needs identified through the CHNA. Carolinas ContinueCARE Hospital at Pineville will utilize its 2017 plan as a guide through 2019 to offer programs that support the health of the community and the mission of the organization.
